When you're register with Amberley Medical Centre you have access to 24 hour Medical and Emergency Care. Download a registration form, pop into the clinic or call one of our friendly staff and we'll mail one out.

** All new patients receive a Free Health Check with one of our experienced Nursing Team upon registration.

Enrolling at Amberley Medical Centre

To enrol with Amberley Medical Centre simply fill in the Enrolment form and return it to reception.

Amberley Medical Centre is a member of the Rural Canterbury Primary Health Organisation which provides funding, subsidised care and special programs for our Enrolled patients. The aim of this funding is to benefit and meet the health care needs of the local community.

Amberley Medical Centre is committed to working with our community and providing you with excellent health care. We encourage our community to enrol with its local Practice to ensure you have continued access to essential local 24 hour medical care right here in Amberley.

How to Enrol with Amberley Medical Centre

  1. Call in and collect an enrolment form from Reception or download our enrolment form.
  2. Please complete page 1 and 2 for each person wishing to enrol
  3. Return the completed form to the Practice and we will process your registration and request your medical notes from your present Practice

Please note: the Ministry of Health now requires proof of eligibility for all new enrolments, including NZ citizens. This can be in the form of a birth certificate, passport, Work/Visitor’s Visa, Community Card. Please speak with one of our Reception team for more information.

Free Health Check for all New Patients

When you register with Amberley Medical Centre you will be invited to book in for a Free Health Check with one of our experienced Nursing Team. This helps us ensure we understand your health requirements (medications, chronic disease management), identify any health issues which may be concerning you and ensure our records are fully updated which means we are better able to meet your health needs in a timely and safe manner.
