Our New Patient Health Check
the first step to creating Wellness Together
We require all newly enrolling patients, aged 18 years and over, to attend a New Patient Health Check with our Health Care Assistant.
Here's why our New Patient Health Check is vital
This one-time check is designed to give you the best start with us. It helps us get to know you and your health needs. It also ensures we have the information needed to provide safe, personalised care.
During your Health Check appointment, we will:
- Record key health indicators (e.g. blood pressure, weight, height)
- Review your medical history
- Check your vaccination status and screening needs
- Set up appropriate recalls and reminders
- Introduce you to our practice systems, including how to book appointments and use the patient portal
- Answer any questions you may have about your health needs or our services
As part of your enrolment and before your New Patient Health Check we'll get you to fill out the New Patient Questionnaire - download it below or there will be a hard copy in the pack at the Practice.
