Enrolment Information

When enrolling at AMC you'll need to come in to the practice.

You'll find us at 6 Hilton Street, Amberley

Please bring the following with you (for each person):

- A completed Enrolment form

- A form of Identification including one with a photo 

(Passport, Birth Certificate, Community Services Card)

-  a Mobile Phone  number - for appt and prescription reminders

- 1 x Individual Email Address  per patient (over 18yrs old)
