Our extensive clinical team works alongside our long term condition patients to provide support and care. This includes medication management, lifestyle advice including nutrition and activity, and support.

We encourage patients to come in for a regular review of their long term condition.

Diabetes

Our Diabetes lead nurse Katie Croft works closely with patients to provide support and care to assist those with Diabetes control. 

Respiratory

Amberley Medical Centre has provided a comprehensive respiratory service for many years, which has been proven to be very successful.



Asthma and COPD can be very debilitating and can reduce your quality of life. Our program is designed for early and accurate diagnosis and appropriate management and patient education.


Amberley Medical Centre operates an accredited, community-based spirometry service which enables early and accurate diagnosis. This is a funded service.

Cardiac Rehabilitation

Cardiac Rehabilitation is offered to people after a cardiac event or surgery. This includes heart attacks, angina, arrhythmia, heart failure and heart surgery. 


Cardiac rehabilitation supports your recovery and ongoing wellbeing. It reduces your risk of future heart events and improves your quality of life.  


We have a long term conditions team to assist our cardiac rehabilitation patients.


Cardiovascular risk assessments are FREE for eligible patients.

We are open Monday to Friday 8am to 6pm with a late night Thursday until 8pm. We also have Weekend Clinics for urgent needs.

We want to support you with a wide variety of services, less travel time and appointment times to suit your family and your lifestyle.
