Our Long Term Conditions team
The AMC LTC Team includes specialist nurses and our practice pharmacist who work closely with our Prescribers.
This team plays a vital role in delivering high-quality, patient-centered care to our patients living with chronic health conditions such as diabetes, heart disease, respiratory disease (asthma, COPD), hypertension (high blood pressure).

What can you expect in a LTC appointment?
The LTC Team has extended consults to carry out regular reviews of your long term condition.
Through longer consultations, patients are given time to discuss concerns in more depth, allowing early identification of complications and preventing unnecessary hospital admissions.
These appointments include :
- medication reviews and
- blood test monitoring
which ensures treatment remains effective and safe.


The LTC team actively remain up to date with the latest treatments, medications, and research, ensuring care is evidence-based and aligned with current best practice.
